Juniors that were scheduled for Granite School District’s statewide administration on April 7 may NOW take the ACT by participating in a national test on ONE of the following dates:
June 13
July 18
September 12
October 24
Vouchers will be mailed to you, based on the test date you wish to use, by completing the ACT Voucher Request Form. (This is a form only for Cottonwood High 11th grade students)

Vouchers should be made available to the ACT Coordinator at Cottonwood High on or about April 20th. There is a May 8 deadline to register for the June 13 test so students wishing to test on that date will need to act fast after receiving the voucher. If you are planning to apply to a college that has early admissions such as the University of Utah or BYU, you will want to plan on testing this summer. Vouchers for the September and October dates will be distributed in mid-August.
Student may NOW choose a date to use a voucher. Students may only receive ONE VOUCHER and will need to register for the test before the registration deadline. Students that have been approved for accommodations or English Language Learner (ELL) support will also receive an accommodation/ELL form in the mail along with the voucher that will allow them to test with accommodations/ELL support on the National Test. THESE VOUCHERS ARE ON A FIRST COME FIRST SERVE BASIS! PLEASE COMPLETE THIS FORM NOW IF YOU WISH TO TEST IN JUNE OR JULY, VOUCHERS FOR SUMMER TEST DATES MAY BE LIMITED. IF YOU REQUEST A VOUCHER BUT DO NOT USE IT FOR THE DATE REQUESTED YOU WILL NOT BE ABLE TO RECEIVE ANOTHER VOUCHER.
